Children ages 6–12 enroll alongside a parent member. Teens 13–17 may enroll on their own, with a parent or guardian signing the agreement. This practice does not accept patients under age 6.
Family discount: households of five or more get 10% off each member’s rate, starting with the fifth member.
Membership is not health insurance. A direct primary care membership covers the primary care services we provide. It is not a health benefit plan and does not satisfy any individual health insurance requirement or mandate under state or federal law. It does not cover hospital care, emergency services, specialists, or prescription costs. Many members pair a direct primary care membership with a high-deductible health plan. We encourage you to maintain separate coverage for care outside this practice.
As of January 1, 2026, direct primary care memberships are HSA-eligible. You can pay for your membership with pre-tax HSA funds, up to $150 per month for an individual and $300 per month for a family.
For many people, that means a $99 membership costs closer to $69 in take-home pay.

You can book a single video visit for an acute problem without becoming a member.

If you join within 30 days, the full visit fee applies to your enrollment.
One-time visits are for new, acute problems only. They don't include ongoing care, chronic disease management, refills, forms, or controlled substances, and they don't establish an ongoing physician-patient relationship. Available to patients located in North Carolina, ages 6 and up.
